Community involvement in our planning is critical. The Community Health Plan combines our independent Community Health Assessment (CHA) with a 5-year Community Health Improvement Plan (CHIP).

Take the TCHAYellowhawk and our partners completed the 2024–2028 Community Health Improvement Plan (CHIP). A CHIP cannot be accomplished without the feedback and perspectives of our community.
In December 2022, Yellowhawk completed our very first, independent Community Health Assessment (CHA). Our Public Health Department leads the CHA and CHIP process.
