Tribal Community Health Assessment
Yellowhawk cares about the health and wellness of our community. To get a better picture of community needs and plan for the next five years, we are asking every person ages 18+ to fill out our Tribal Community Health Assessment (TCHA).
It is estimated that the survey takes 40–60 minutes to complete. The survey will ask personal health questions — we assure you that the information will remain confidential and will not be individualized.
The CTUIR is one of only a handful of Tribes that conduct our own Tribal Community Health Assessment. Help us continue this important work so that we may plan for our futures and see our growth along the way.
