Medical Records receives requests via fax, USPS, FedEx, and UPS. If you wish to hand deliver your request, please drop it off at the medical reception desk during business hours.
Turnaround Times
Requests are processed within five days of receipt, depending on requester type. Routine continuation-of-care requests are typically processed within one working day; urgent continuation of care within 30 minutes.
